  Please read these instructions carefully before submitting.  

1. Students are not eligible to nominate themselves for the competition.  This form must be filled out by a faculty member currently working at the school at which the student is enrolled.
   
   
2. The ISC can only accept a total of 3 student nominees from each school.  However, those three nominees can come from faculty members from any department.  (For example, if your school has a sculpture department and a ceramics department, your faculty can decide how many students from each department to nominate as long as you do not exceed 3 total student nominees.)
   
   Please check with other professors in your department (and, if applicable, other departments who might want to nominate students) to ensure that only one form is submitted for your school.  The ISC will endeavor to contact you if conflicting forms are submitted.
   
   
3. MFA and BFA programs at the same school can only nominate a combined total of 3 students, even if the programs have two separate member numbers.
   
   
4. Schools with art programs at completely separate campuses may nominate 3 students per campus, provided each campus has a member number.  Example:  University of Wisconsin, Madison vs. University of Wisconsin, Milwaukee.
